She is survived by: her mother Pam Bolden (Rusty); her husband Dylan Smith; her children, Silas and Abigail; her parents-in-law, Kevin Smith (Audra) and Kim Smith; her siblings, Kim Harris (Levi) and Allen Townsend (Joyi); her siblings-in-law, Morgan Smith, Hannah Barbour, Hunter and Zachary Barbour; and her nieces and nephews, Levi, Kallii, Andii, Cheyanne, Lakyn and Nolan.

In lieu of flowers, memorials may be made to Levine Children's Hospital or the Autism Society of North Carolina in Chelsea's name.
