He is survived by: his wife Deborah Mathis Cook; his daughters, Lisa Cook Setzer (Chris) of Maiden and Amanda Cook Petty (Wesley) of Conover; his grandchildren, Kaylee Setzer of Maiden, Mackenzie Petty of Conover and Caleb Petty of Conover; and his brother Ralph Cook (Betty) of Hickory.
