He was predeceased by: his parents, Johnny Perkins, Sr. and Mary Elizabeth Smith Perkins; and his brother Donald Perkins. He is survived by: his wife Jean Perkins; his sons, Dwayne Perkins (Tammy) and Timothy Perkins (Mary); his sister Laura Bumgarner; and his stepchildren, Denise Johnson, Shannon Church and James Johnson. He is also survived by five grandchildren and seven great-grandchildren.
