He was predeceased by: his parents, Autus Frank Cook and Hettie Pearl Cook; and his brother Jimmy Frank Cook. He is survived by: his wife Elizabeth; his children, Julie Paxton of Hickory, Ralph Cook, Jr. (Terrie) of Hickory and Jeremy Cook (Tosha) of Hickory; his grandchildren, Jennifer, Tyler, Gabe, Savannah and Dallas; and his great grandchildren, Aaliyah, Ethan and Shantel. He is also survived by his friends and family.

Memorials may be made to the Hickory Seventh Day Adventist Pathfinder Club.
