He is survived by: his sister Elaine Cook Ostwalt; his wife Kay Branton Cook; his children, Mark Cook (Kim Clark) of Lenoir, NC, Stephen Cook of Statesville, NC and Allison Bolick (Toby) of Statesville, NC.; his grandchildren, Jonathan Cook (Emily), Jordan Cook (Amanda), Meleah Cook, Ty Bolick, Megan Anderson (Cody) and Michelle Lambert (Peter); his great grandchildren, Madelyn Cook, Dawson Cook, Gwendolyn Cook, Abram Anderson, Adler Anderson, Hudson Lambert, Ansley Anderson and Henley Lambert; his sister Elaine Cook Oswalt (Lathan); and his sisters-in-law, Katherine Cook and Norma Rupard.
