He was predeceased by: his parents, William Avery Hayworth, Sr. and Treva Stanley Hayworth; and his wife Marilyn Joyce Sumner Hayworth. He is survived by: his son Lee Hayworth (Beth).

In lieu of flowers, memorials may be made to Hospice of the Piedmont, 1801 Westchester Drive, High Point, NC 27262.
