He is survived by: his wife Annie Mauthé; his daughters, Cynthia Mauthé, Catherine Mauthé and Janet Frazier; his son-in-law Matthew Frazier; his grandchildren, Jordan Seymour, Joseph Frazier and Lily Frazier; his goddaughter Sandra Collins (Greg Collins); his sister Melody Douglas; his nieces, Julie, Becky, Sami, Tiffany and Liz; his nephew Nick; and his siblings-in-law, Kay, Margaret (Drew), John (Sylvia) and Michael.

In lieu of flowers, the family would prefer donations be made to "The Gary Sinise Foundation" or "St. Jude Children's Research Hospital".
