He is survived by: his wife Rose Buonocore Gallagher; his daughters, Carol Hayworth (Douglas) of Jamestown, Peggy Leyden (Dennis) of Greensboro and Janet Phillips (Gregory) of East Aurora, NY; his grandchildren, Benjamin Leyden (Helen), Sarah Privott (Cameron), Megan Hayworth (Leonardo), Emily Hayworth, Jessica Phillips and Hannah Phillips; and his great grandchild Thomas Leyden.
The family asks with gratitude that memorials in Tom's name be made to the Cystic Fibrosis Foundation.
