He was predeceased by: his parents, Frank Puvak and Katherine Puvak. He is survived by: his granddaughter Abby; his wife Cindy; his children, Daniel Puvak (Aimee) and Jaimie Wilson (Matthew); and his grandchildren, Grace, Jacob Wilson, Madeline, Abigail and Isabelle Puvak. He is also survived by many extended family members and friends.
