He was predeceased by: his father Timothy Cook. He is survived by: his wife Tiffani Cook; his daughter Nevaeh Cook; his siblings, Jeremy Cook (Codie) and Jessica Hamilton (Jeremy); his parents-in-law, Belinda Jones (Romus Jefferies) and Jeff Leigh (Pam); and his grandmother Nancy Brown. He is also survived by numerous nieces and nephews.
