He was predeceased by: his parents, John Conrad Cook and Gleta Morteith Wilson; and his son Christopher Cooper. He is survived by: his wife Polly Houck Cook; his son John Paul Cook (Kelly) of Lexington; his sister Glenda Johnson of Lexington; his grandchildren, Eli Balies Cook, Asher Edward Cook, Cecila Marie Cook and Christopher "Lee" Cooper (Serenity); his nephew Matthew Catlin (Lydia); and his nieces, Summer Canovai (Lance), Ciera Mesimer (Justin) and Brittany Reynolds.
