He is survived by: his parents, Michael Dean Ballard of Vale and Melinda Heavner Ballard of Vale; his daughters, Madilyn Ballard of Crouse and Makayla Ballard of Crouse; his sister Mindy Ballard of Lincolnton; his fiancé Kelly Carpenter; and his partner's child Maxon of Crouse.
