He was predeceased by: his parents, Loy William Cook and Margaret Griffin Cook; his daughter Rita Cook Newton; his daughter-in-law Syntha Caldwell Cook; his grandson Todd Newton; his great great grandchildren, Alani and Alaya Bowman; and his siblings, Clara Cook Meacham, Ruth Cook Winebarger and Lester Loy Cook.
He is survived by: his wife Carolyn Day Cook; and his children, Michael T. Cook of Dallas, NC, Donna Cook Gardner of Walla Walla and Marcia Cook Eden of Lincolnton. He is also survived by six grandchildren; ten great-grandchildren; and two great-great grandchildren.
