He was predeceased by: his mother Betty Jean Beal Ballard. He is survived by: his father Jerry Donald Ballard; his siblings, Mike Ballard (Rande), Ken Ballard (Tammy) and Donna Ballard; his daughter Crystal Brooks (David) of Lincolnton; and his grandchildren, Chloe and Caleb Brooks.
