He is survived by: his wife Doris Cook (Fife); his children, Cathy White (Kevin), Frank Cook and Michelle Davis (Jeff); his grandchildren, Crissy Clark (Mason), Katie McClendon, Frank W. Cook, Karlee Cook, Hayley Davis, Luke Davis and Mackensey Davis; his great grandchildren, Brian Hurt and Savannah Hurt; and his sisters, Linda Pohutsky and Eleanor Clark.
In lieu of flowers, the family kindly requests that donations be made in Frank's memory to the American Heart Association.
