He was predeceased by: his parents, David Dawson Cushman and Donna Jean Phillips Cushman. He is survived by: his wife Debbie Shirlen Cushman; his children, Adrian Cushman, Sherwin Avila, Kaitlyn Burkley, Gloria Mazaheri and Maria Janicek; and his siblings, Bruce Cushman, Teresa Hooker, Nancy Flamand and Cindy Jarnagin. He is also survived by many grandchildren and great-grandchildren.
