He was predeceased by : his wife Shirley Jean Evans Cook; his parents, John M Cook and Minnie McClain Cook; his daughter Sherry Ann Cook; and his siblings, Richard Cook, Clearance Robert Cook, Timothy Cook, Daniel Cook, Easter Barberee, Dorothy Rish and Nancy Adkinson Pertalla.

He is survived by : his sisters, Martha Mendaz and Daisy Brinson; his daughters, Crystal Williamson (Chad) and Tara Williamson (Ed); and his grandchildren, Brett Dorsch, Marcus Dorsch, Stephen Dorsch, Chandler Simmons, Summer Williamson, Tyler Williamson and Lakyn Williamson. He is also survived by five great-grandsons and three great-granddaughters.
