He is survived by: his parents, Matthew Cook and Meredith King Cook; his sister Elizabeth Cook; his grandparents, Gerald, Vickie Cook, Maurice and Carolyn King; and his cousins, Adam, Amy, Molly Belle, Chloe Cook, John, Mandy, Caroline and Sally Wilsey.
