He was predeceased by: his father Benito Velazquez. He is survived by: his wife Marty Velazquez; his children, Danny, Elisha Velazquez and Nicole Velazquez Hull (Michael); and his grandchildren, Isabelle, Ivan, Sabrina Velazquez and Remy Hull.

In lieu of flowers, the family would appreciate donations to help with funeral expenses.
