He was predeceased by: his parents, Charles Louis Perkins and Rutha Swabb Perkins; his brother Doug Perkins; and his nephew Patrick Perkins. He is survived by: his wife Teresa Perkins; his sons, Michael of California and Joshua of Ohio; his brothers, Dennis Perkins (Jane) of Bradford Ohio and Daryl Perkins (Sherrie); and his grandsons, Michael, Joshua and Gabriel.
