He was predeceased by: his son-in-law Steve Acord; his parents, Foley Cook and Orlie Rose Hinkle Cook; and his brother Phillip L. Cook of Cleveland, TN.. He is survived by: his wife Jonda Lee Keathley Cook; his daughters, Becky Donahue (Ed) of Rolesville, NC, Teresa Beavers (Bruce) of Clayton, NC and Debbie Acord of Rolesville, NC.; and his siblings, Pat Brown of Man, WV, Lloyd Cook (Bonnie) of Goldsboro, NC, Dale Cook (Marty), Charles Cook (Pattie) of Danville, WV and Ben Cook of Beckley, WV. He is also survived by seven grandchildren, and thirteen great-grandchildren.
