He was predeceased by: his parents, John Simpson Slate, Sr. and Nell Hunter Slate; his wife Caroline Marcinkowski Slate; and his son Kevin Slate. He is survived by: his son Keith Slate (Sherry); his grandchildren, Savannah Lyons (Richie), Christin Slate, Ryan Slate, Landon Slate and Devin Slate; his great grandchildren, Carter Slate and Lavender Lyons; his brother Bobby G. Slate (Cathy); and his nieces, Ashli Nicks, Morgan Martin and Brooke Bliss.

In lieu of flowers, memorials may be made to: Blackwater Baptist Church, 112 Church Ln., Mt. Airy, NC 27030.
