He was predeceased by: his parents, V. S. "Buck" Lyerly and Juanita Lyerly; his brother Jake Lyerly; Charles Donald "Don" Lyerly and Mark Christopher Lyerly.
He is survived by: his wife Sue Waters Lyerly; his son Dr. Matthew Scott Lyerly (Gena); his grandchildren, Caroline and Jacob Lyerly; and his sister Nancy Readling. He is also survived by many nieces, nephews; and great nieces, great nephews.
In lieu of flowers, memorials may be made to the Nazareth Child & Family Connection which was founded by his grandfather, Jacob Martin Luther Lyerly.
