He was predeceased by: his parents, James P. Polk of McRae and Florence E. Polk of Denver, Colorado.; and his daughter Nicole Danielle Polk of Long Beach, California.. He is survived by: his wife Kaye Cline Polk; his stepchildren, Burt Broughton (Nicole) of Gastonia, NC and Barbie Sprouse of Shelby, NC; and his grandchildren, Clay Whitworth, Alyssa Broughton, Hunter Ledford and Weston Sprouse.
