He was predeceased by: his wife Janice Lewis. He is survived by: his sister Deanna Cook; and his children, Thomas W. Lewis, III (Renee Cardone), Donna Amoroso (William), Debbie Lewis-Manni (Robert Manni) and Donald Lewis (Christina Lewis). He is also survived by grandchildren, great-grandchildren, nieces and nephews.
