He was predeceased by: his father John Slezak. He is survived by: his wife Stephanie Cashman; his children, Elizabeth Gray (Kevin), Kaitlin Jutras (Conrad) and LeChele Miller; his grandchildren, Isabella, Ben Gray, Carson and Parker Jutras; his mother Vivian Slezak; and his sisters, Bobbi Ammann and Lynn Cashman. He is also survived by many nieces and nephews.
