He was predeceased by: his parents, Ollie Rifton Cook and Gladys Ann Cook. He is survived by: his wife Becky Rivenbark Cook; his children, Thomas Cook (Jessica) of Clinton, NC and Betsy Cook of Warsaw, NC; his grandson Colton Ray Cook; and his siblings, Jo Ann Barrett and Jeff Cook (Eva).
