He was predeceased by: his parents, Henry W. Kostelecky and Helen B. Kostelecky (Hatzenbiler); and his sister Shirley Korpi (Vic). He is survived by: his father-in-law Tony Pavlicek; his brother-in-law Gordy Pavlicek (Linda); his child Kerry; his wife Audrey (Pavlicek); his children, Jim Kostelecky (Kerry), Cindy Kostelecky (Kerry Paulson), Virginia and Nancy (Greg); his grandchildren, Henry, Audrey Paulson, Gabe (Anna), Savannah and Elizabeth Sowards; his great grandchild James Sowards; and his siblings, Ted Kostelecky (Marge), Patty Sibet, Marilyn Dean (Ron) and Ron Kostelecky (Rita). He is also survived by numerous cousins, nieces, nephews and extended family.
