He was predeceased by: his parents, Percy M. Lewis and Charlotte M. Lewis (Cook); and his wife Patricia Jane Nuttall. He is survived by: his children, David, Susan, Marti, Mary and Ruth. He is also survived by his four children, six grandchildren and two great-grandchildren.
