He was predeceased by: his parents, Donald Leo Hertz Sr. and Marian Hertz (Jensen); his wife Kathy Barr Glass; and his sibling's children, Jennifer Erickson, John Erickson and Henry Weber, Jr..

He is survived by: his former wife Vicki Homuth; his children, Duffy, Suzanne Brucker (Henry), Aitkin, Leah Staahl (Gus), Donald Leo III, Fountain Hills, Dustin (Jodi), Franklin and Tenn; his grandchildren, Jesse Helbling, Victoria Harris (Evan), Maple Grove, Alexandra, Joseph Brucker, Aitkin, Jonah, Joshua, Joseph Hertz, Franklin, Tenn, Griffin and Ian Staahl; and his siblings, Linda Hertz, Brainerd, Karen Weber (Henry), Debbie Erickson (Roger), Gilbert, Tom Hertz (Jill) and Bill Hertz of Denver, N. C.. He is also survived by ten nieces and nephews; and many grandnieces and grandnephews.

The family has designated the Michael J. Fox Foundation for Parkinson's Research for memorial contributions.
