He was predeceased by: his parents, Fred Prochaska and Lillian Prochaska (Rozanek); his brothers, Lester and Elmer; his sister-in-law Lorene Prochaska; and his stepsisters, Ilene and Lydia Prochaska.
He is survived by: his nephew Larry Prochaska of Columbus; his great-niece Morgan Prochaska of Omaha; his cousin Melvin Prochaska of Malmo; his sister-in-law Deloris Prochaska of Madison; and his sibling's children, Sharon, Darrell, Diane, Duane, Lori and Kevin.
