She was predeceased by: her parents, Charles Sr. Provenzo and Gwendolyn Provenzo (Hogue); her husbands, Leonard Smith and Quay Irwin; her sons, Leon and Paul Smith; and her siblings, Joyce Childs and Charles Provenzo Jr..
She is survived by: her daughters, Carol Frase (Paul) of Beatrice and Lila Stewart (Bill) of O'Neill, NE; and her sister Edna (Ronald). She is also survived by 8 grandchildren, 23 great- grandchildren and 3 great-great grandchildren.
