He was predeceased by: his mother Joann Chudomelka (Kopietz); and his granddaughter Bailey JoAnn Mathis. He is survived by: his children, Brenda Mathis, Lacey Chudomelka (Badi), Jena Chudomelka, Kylie Chudomelka, Hagin Frink and Octavian Frink; his grandchildren, Karsyn Jones, Hayden Frink-Mathis, Javon Chudomelka, Kaleigha Pierre, Ivy Baniab and Dwayne Chudomelka; and his sisters, Jo Hoge (Leland), Holly SchollNephews-Lucas Hoge (Becca, family: Aaron Dodge, Jodran Hoge) and Logan Dodge.
Memorials may be directed to the family for future designation in Doug's name.
