She was predeceased by: her mother Frances Dubas (Szewczyk); and her husband Alphonse Kozak. She is survived by: her children, Dorothy Carlson (Gale) of St. Edward, NE, Dennis Kozak of Webster, SD, Ray Kozak (Gina) of Columbus, NE, Tom Kozak (Kelly Burge) of Columbus, NE., Paul Kozak (Marge Prokupek) of Columbus, NE and Barbara Smith (Jim) of Columbus, NE; and her daughter-in-law Angie Ahrens (Many). She is also survived by Many Grandchildren, Great Grandchildren and Great Great Grandchildren.
