He was predeceased by: his parents, Althea Koolen and Martin Koolen III; and his sister Christy Koolen. He is survived by: his son Kelley Michael (Erin); his wife Jacquie; his son Martin V (Annie); his grandsons, Martin VI and Calvin; his son Jeff (Jennifer); his grandchildren, Shea, Brady, CJ, Alaina and Kaylee; and his siblings, Kent Koolen and Gretchen Wagner. He is also survived by numerous nieces and nephews.
In lieu of flowers, memorials may be directed to the Fremont Family YMCA to support youth programs and the member financial assistance program.
