He was predeceased by : his parents, Edward R. Horacek and Sarah L. Horacek (Brubeck); his former life partner Patricia Carter; his wife Rogene (Butch Stephens); and his granddaughter Macy Carter.
He is survived by : his children, David Horacek (Patricia) of Florida, Mary Ann LaFleur (Phil) of Colorado, Diana Sample (Ron) of Nebraska and Carol Shima (Stephen) of Colorado; his stepson Gary Carter of Nebraska; and his sister Gail Kimminau of Nebraska. He is also survived by 13 grandchildren, 14 great-grandchildren as well as many nieces, nephews, other relatives and friends.
