He was predeceased by: his parents, Harold Simmons and Gertrude Simmons; his sister Eleanor; and his mother-in-law Mary Lou Percival. He is survived by: his wife Melody Percival; his children, Matthew (Shannon) of Davey, NE, Jason (Alison) of Lincoln, NE. and Alicia of Longview, TX.; his grandchildren, Kole Reece, Kole Russell, Kaileigh, Kamarin, Lizzy and Maddy; his great grandchildren, Jaxson, Mason and Everleigh; his brother-in-law Kurt Percival (Angela); his nieces, Belinda and Amy; his father-in-law Mel Percival; and his friend Dave.

In lieu of flowers, the family requests memorial contributions in David's name be made to St. Marks United Methodist Church Youth Group.
