He was predeceased by: his mother Linda Panek (Homolka); and his brother Jason. He is survived by: his father John Panek of Madison; and his children, Madden and Olivia Panek of Norfolk.
The family is asking that in lieu of flowers, memorials be designated to the family for purchase of a cemetery marker.
