She is survived by: her daughter Renee Forman of Norfolk; her grandchildren, Diego Garcia of Norfolk, Kamila (Antonio Aguilar) of Norfolk and Marco Carrion of Norfolk; her great grandchildren, Cam'Ron, Winona, Farrah and Jose (Junior); her siblings, Jerome Forman (Sandra) of Verdigre, Donald Forman (Connie) of Verdigre, James Forman (Kathy) of O'Neill, Dennis Forman of Verdigre, Noreen Conrad of Norfolk and Betty Boggs (Kevin) of Winnetoon; and her brother-in-law Lyle Thelen of Creighton. She is also survived by nieces and nephews; and her very special friend.
