He was predeceased by : his mother LaDonna Cook; and his wife Linda Cook. He is survived by : his father Rich Cook; his children, Shanandoah Scott (Tim), Breanna Cook (Tyson Courter), Trevor Cook (Naomi), Damon and Brendin; and his sister Suzette Casillas. He is also survived by several grandchildren and great-grandchildren.
