He was predeceased by: his parents, Gene Collins and Veva Collins (Schwinderman); his grandchild Elisia Collins; and his grandparents, Edward and Evelyn Collins (Marshall).

He is survived by: his nephew Cory Chapman; his wife Kathleen Collins; his children, Alicia Weiland (Skot) and Jason Collins; his grandchildren, Tasia Bochnicek (Donald), Drew Weiland (Amanda Hostert), Logan Weiland and Austin McIntyre; and his siblings, Chris Chapman (John) and Gary Collins (Cindy). He is also survived by nieces, nephews, and other family members.

His family asks that memorials be directed to the Epilepsy Foundation.
