He was predeceased by: his parents, John Costanzo Jr. and Ellen Costanzo; and his brothers, Mike Costanzo and Rick Costanzo. He is survived by: his daughters, Jennifer Costanzo (Lissa) of Ft Worth, TX and Melissa Costanzo of Omaha; his grandchildren, Jameson Bryan and Maisie Bryan of Delcambre, LA; and his nephew Anthony Costanzo (Brienna).
