He was predeceased by: his father Gary Lewis; and his grandparents, Alice, Jack Kaya, Harold and Ida Lewis.
He is survived by: his mother Carol Mudra (Rudy); his daughter Kylee Lewis; his grandson Orion Myers; his siblings, Mike Lewis and Michelle Gruber (Darrell); his nieces, Sierra Boots (Devin) and Skye Harlow; his aunts, Jackie Shindo and Marilyn Hespen (Todd); and his stepsister Derinda Eagleton. He is also survived by his extended family and many good friends.
