He is survived by: his former wife Penny Teply (Hill); his sons, Matt, Lindsey Teply, Patrick Teply and Tanner Teply; his grandchildren, Tate and Edisyn Teply; his siblings, Sam (Kristi Teply) and Cindy (Steve Balsom); and his nieces and nephews, Buck, JJ Teply, Logan (Shari Mohr), Jessie (Dale Liskey), Chris, Stephanie Hughes, Dean, Jennifer Hughes, Tricia Hughes (Shane), April (Hughes) and Mike Kinninger.

Memorials in Dave's memory are suggested to St. Jude Children's Research Hospital.
