She was predeceased by: her parents, Frank Kotas and Sylvia Kotas (Zoubek); her husband Myron Hayek; her parents-in-law, Lumir and Evelyn Hayek; her son-in-law Darin Keller; her brothers, Gerald and Roger Kotas; her sisters-in-law, Barb Kotas and Marva Kasl; her nephews, Bobby and Brad Kasl; and her niece Jean Essman.
She is survived by: her children, Wendy Keller, Doug Hayek (Hillary) and Daryl Hayek (Roxy); her grandchildren, Seth Keller (Jordan), Zack Keller (Meredith Hoffman), Logan Hayek (Russell Korinek), Rylee Zeleny (Sam), Kaylee Slepicka (Dan), Collin Hayek (Tara Schroeder) and Cooper Hayek; her great grandchildren, Lucy Keller, Roayn Clausen-Zeleny, Rextin Zeleny, Dax and Josie Slepicka; her brother Melvin Kotas; and her siblings-in-law, Lois Kotas, Carol Kotas and Bob Kasl. She is also survived by many nephews, nieces, relatives and friends.
