He was predeceased by: his parents, Paul Forkus and Wilma Forkus (Grande); his son Michael "Donny" Forkus; and his sister Gail. He is survived by: his wife Marie "Rio" Forkus (Thomas); his son Paul J.; his daughter-in-law Renee Forkus of Atkinson; his daughter Deborah; and his children-in-law, Brad Valente of Lakeville, MA and Susan Forkus of Bryant Pond. He is also survived by eleven grandchildren, eight great- grandchildren, and several nieces, nephews and cousins.
