He was predeceased by: his wife Joyce J Kozak (Deveau). He is survived by: his daughter Karen McNamara of Haverhill; his stepdaughters, Lynne Murray and Gail Murray; his brother Peter; his sister-in-law Laurie Kozak of Amherst, NH; and his sisters, Alice (Martin Viega), Victoria LaCourse of Nashua, NH and Cheryl (Dennis Long). He is also survived by four grandchildren and one great-granddaughter, and several nieces and nephews.
