He is survived by: his wife Judith Falkowski (Chartier); his daughters, Colyn Belter (Mark) and Erinn Falkowski; his grandchildren, Abygail and Jackson Forbes; his sister Susan Miller (David); his cousin David Carlton (Jolene); and his friends, Bernard and Kathleen Baronas. He is also survived by several nieces, and nephews.
