He is survived by : his wife Gloria Polizzi; his children, Dale Jack (David), David Polizzi (Renee) and Donna Hawkes (Jason); his sister Lillian Rae; his grandchildren, Matthew Jack, Brittany Cutrell, Hannah Shultz, Brad, Austin Polizzi, Lauren Pierce, Madison, Makayla and Parker Hawkes; and his great grandchildren, Cooper, Blake, Evelyn, Ava, Annie, Vincent, Oliver and Joelle.
