He was predeceased by: his parents, Frank Cook and Mary Cook (Gormley); and his sisters, Carolyn Schneider and Joan Adriaanse. He is survived by: his wife Victoria Cook (Miele); his children, James D. Cook and Patricia Ann Cushing (Jeffrey); and his grandchildren, Robert Cook, Patrick Cushing and Dana Olverson. He is also survived by 3 great grandchildren.
